Once your eligibility is confirmed, we can tell how much money you can use to arrange and buy your support.

You have some options such as,

Eligibility

The Adult Disability Service offers a range of support options. This is for people who are eligible, aged 18 or older and have:

  • a learning disability

  • a physical disability

  • a long term condition such as motor-neurone disease

  • an acquired brain injury following a stroke or accident

How we can support you

We take time to get to know you and find out how you want to be supported.

We help you create a plan to meet your needs for things such as,

  • living in a place that meets your needs and you are happy in

  • gaining new skills and pursuing your interests

  • taking part in a range of social and leisure activities with people of your choice

  • maintaining your health and wellbeing

  • developing your own support plan

  • working out who you would like to support you

  • staying safe

  • meeting new people and making friends.

Our services in your community

Our services are located within your local community and facilities. We offer opportunities for you to learn and regain skills to live the life you want.

Ways we can support you

  • your carer to take a break

  • get ready for paid employment.

The Enablement Service

They will support to develop your confidence and skills to be more independent.

Shared Lives Service

We can support you to live with a family. Our Shared Lives Service is family-based care provided by individuals and families. This service supports adults to develop their strengths and abilities.
