Get support for alcohol and drug rehabilitation

We offer support for you or your loved ones needing alcohol or drug rehab services. Simply complete out our referral form to receive support.

Check what you need before you start 

The information you give us will help us find the right support.

This could be support provided by us or one of our partner organisations. For example,

  • our community drug and alcohol service

  • community group

  • voluntary group

  • faith group

Information you need to provide

We will need to know:

  • personal details, such as your name, date of birth and address

  • support you receive from other people

  • medical conditions or medication

  • General Practitioner (GP) details.

How to complete the referral form

  • You need to have an email address to register for an adult social care account 

  • we send a security authentication code to your email address when you register

  • after you have registered, you can complete and submit the referral form. You can also save and come back to it later. 

  • we will email you a different security authentication code each time you sign in to your account. The code is valid for 10 minutes and can only be used once. 

What happens next

A social worker will contact you for a phone discussion. This is followed by an initial appointment.

What to expect at your initial appointment

Your social worker will talk to you about a range of treatment and post detox options.

These include, but are not limited to:

  • residential rehabilitation 

  • non residential treatment

  • supported housing. Drug and alcohol-free housing to support you to stay substance free whilst living in the community

  • step-down services from recovery support groups 

  • mutual aid support 

After your appointment

Your social worker can offer you support and advice around universal services. This could be for debt management, tenancy or housing issues or victim support services.

To plan you support we may work together for other services and support. This can include:

  • physical and mental health services

  • community substance misuse services

  • criminal justice services (or other services that are applicable to you.)

Types of residential and non-residential rehabilitation

We will ask you to have a financial assessment for services to identify if you could pay for some of your support.
