Arranging your own care

You can arrange care for yourself, friends and family without contacting us.

If you think adult social care could support you, a family member or friend you can request a referral.

A referral will include a full needs assessment if we think you may need social care support.

This will make it easier for us to consider your needs and see how we can support you.

If you have already submitted a request for support, use this form to tell us something has changed.
