Jasper GUNTON was born and died on the 21st December 2023 at his home address. He was born following a pregnancy which had been concealed and had not benefitted from ante-natal care. After his birth, Jasper did not receive medical attention and he was taken to bed with his parent who fell asleep with baby Jasper on their chest. In the morning, Jasper was found unresponsive and despite resuscitation attempts by paramedics, was pronounced dead. It is not possible to determine what caused Jasper's death which occurred in the context of a pregnancy and birth without medical assistance.

HUNT, Vincent John
|
Vincent John HUNT died on the 10th January 2025 at the Royal Preston Hospital as a consequence of blood loss. Mr HUNT was an inpatient for the purposes of receiving dialysis due to acute kidney injury, chronic kidney disease and a background of multiple myeloma. Mr HUNT was staying in a side room due to infection. On the evening of the 10th January 2025 Mr HUNT was found unresponsive in a pool of blood. A femoral line which had been inserted to facilitate dialysis was dislodged, resulting in the blood loss. It is not possible to determine how the line became dislodged.

McCLUGGAGE, Gary
|
Gary MCCLUGGAGE died on the 11th May 2025 at the Royal Blackburn Hospital as a consequence of decompensated heart failure. Mr MCCLUGGAGE had a number of health conditions and lived in sheltered accommodation. He had not been seen by staff since late April 2025. He was taken to hospital on the 6th May 2025 when he was found after intervention by his brother to be very unwell and stuck in his chair for a number of days. He was taken to hospital where despite treatment he died.
