29/09/2025 29/09/2025 MORRIS, Antoinette Marie Antoinette Marie MORRIS died on 15 March 2025 at Royal Blackburn Hospital, Blackburn in Lancashire. Following a history of abdominal pain and initial diagnostic tests, Miss MORRIS was admitted to hospital on 11 February 2025 and was diagnosed with dilated common bile duct. Various percutaneous transhepatic cholangiogram (PTC) procedures were attempted to treat the bile duct before a successful PTC was completed on 20 February 2025, and a drain was inserted. Further diagnostic tests were undertaken prior to discharge on 27 February 2025, including biopsy. The tests confirmed a diagnosis of pancreatic adenocarcinoma. Miss MORRIS was readmitted on 5 March 2025 following a deterioration in her condition. A further PTC was undertaken during which it was noted the drain had started to dislodge. The drain fully dislodged during the procedure, caused in part by the procedure, and it was not possible to insert a new drain. This resulted in a significant worsening of infection and sepsis. The PTC procedures contributed to the development of sepsis. In addition, a delay in the final PTC procedure also contributed to her death.

30/09/2025 30/09/2025 TURNER, Daniel Paul Daniel TURNER was found deceased in his flat in Preston Lancashire on the 8th December 2022. Daniel's cause of death was blunt force head injuries. It is not possible to determine exactly how Daniel sustained these injuries however it is clear that there were two separate episodes causing him two separate head injuries, the first taking place likely around one week prior to his death and the second taking place around 2-3 days before his death. Mr TURNER was last seen alive on the 7th December 2022 and he was known to misuse drugs.

01/10/2025 01/10/2025 FINLEY, William Brian On 15th January 2025 William Brian FINLEY, known as Steve, fell and suffered a periprosthetic left hip fracture in his room at Chorley Hospital. It was surgically repaired on 24th January 2025 at Royal Preston Hospital, following which he developed deep infection involving the metalwork and hip replacement, which is a recognized risk of this surgery, leading to his deterioration and death on 22nd March 2025 at St Catherine's Hospice, Preston.
