07/10/2025 07/10/2025 BOWKER, Lotus Nathan Lotus BOWKER, a 15 year old boy, jumped into the river Garstang whilst playing with friends on the 17th May 2025. Lotus could not swim and got into difficulty in the river where he was submerged for an unknown length of time. Lotus was rescued by a bystander and resuscitation was commenced. He was taken to Royal Manchester Children's hospital where, despite efforts to resuscitate him, he died on the 18th May 2025 from drowning. Lotus had Development Learning Delay which affected his communication and his ability to weigh up risks and probably contributed to his death.

08/10/2025 08/10/2025 MACDONALD, Malcolm Kenneth Malcolm Kenneth MACDONALD died on the 29th June 2025 at his home in Colne, Lancashire from pneumonia which was a consequence of respiratory issues likely caused by his occupational exposure to asbestos and smoking related lung changes. 
08/10/2025 09/10/2025 STUDHOLME, Adrienne Caroline Adrienne Caroline STUDHOLME died on 23 September 2023 at Royal Blackburn Hospital, Blackburn in Lancashire. Adrienne underwent an elective left nephrectomy on 10 September 2023 complicated by abdominal wall haematoma requiring a further operation on 11 September 2023 before being discharged. She was readmitted on 20 September 2023 at around 3.05 hours with epigastric pain and seizures. Diagnostic checks completed later that afternoon identified spontaneous splenic haemorrhage and rupture (a known complication of nephrectomy) which were operated upon at 18.30 hours, after which she had a myocardial infarction. Despite treatment over the next three days, she did not recover. Her death was contributed to by a delay in diagnosing and treating the splenic rupture.
