09/12/2025	09/12/2025	MULHOLLAND, Margaret	Margaret MULHOLLAND died on the 13th November 2025 at Royal Preston Hospital, having asphyxiated as a result of food material which became lodged in her airway whilst she was eating. Mrs MULHOLLAND'S pre-existing medical conditions, which included a previous ischaemic stroke and dementia, also contributed to her death

10/12/2025	10/12/2025	STRETCH, John William	John William STRETCH died on the 17th February 2025 as a result of a spontaneous rectus sheath haematoma in Preston Hospital. Mr STRETCH was admitted in January 2025 with sepsis caused by a diabetic foot infection. There was a missed opportunity to recognise and treat the foot infection before it became a life threatening emergency requiring emergency debridement and amputation of four toes on the 11th January 2025, which likely contributed to Mr STRETCH'S ongoing frailty and subsequent death. Mr STRETCH developed a spontaneous rectus sheath haematoma on the 16th February 2025. An angioplasty was performed on the 17th February 2025 in the knowledge the rectus sheath haematoma was not actively bleeding or requiring intervention at that time. Mr STRETCH deteriorated shortly after the angioplasty with a catastrophic bleed at the same site as the rectus sheath haematoma which had developed the previous day
