Peter DIXON died on the 11th June 2025 at the Royal Blackburn Hospital as a result of a spontaneous sigmoid perforation of his colon. Mr DIXON had been admitted to hospital on the 27th May 2025 for planned surgery to repair aortic aneurysms. The surgery took place with some complications and a return to theatre was necessary the same day. Following this, Mr DIXON commenced a slow recovery. He suffered with pain, was diagnosed with COVID-19 and he also suffered a heart attack. He then developed a perforated colon but Mr DIXON was now too unwell to survive a further operation. His care was re-orientated to palliative prior to his death.

ALSTON, John Graham
|
John Graham ALSTON, who was better known as Graham ALSTON, died on the 8th of November 2022 at the St Catherine's Hospice, Lostock Hall, Preston as a consequence of injuries he sustained after he was punched to the face on the 2nd November 2022. Mr ALSTON was a resident at a specialist dementia care home due to a diagnosis of dementia. Whilst at that home, he was punched to the face by another resident who was significantly younger than Mr ALSTON and had impaired cognition. The other resident had been inappropriately placed at the care home, in part due to incomplete assessment documents being shared with the home via an unsuitable discharge pathway. Concerns were quickly raised regarding the suitability of the resident's stay at the home but a new placement was not able to be identified. On the 2nd November 2022, at least three staff members were with the resident when Mr ALSTON entered the communal space and touched the resident who responded with violence towards Mr ALSTON. None of the staff members observed Mr ALSTON enter the room or approach the resident and so no staff member prevented the incident from occurring. It was known that the resident presented a risk of violence to other residents and had used violence against Mr ALSTON in the past.

|
Sheila SHUTTLEWORTH sustained a fractured neck of femur following a fall in her home and was taken to hospital to undergo surgery which was performed on the 3rd February 2025. During her admission she had impulsively attempted to ligature. She was discharged to Meadowfield Care Home for rehabilitation and during her admission she had expressed some suicidal thoughts and unmanaged pain. There were missed opportunity to escalate these. She was found deceased on the 17th February 2025 and toxicology revealed fatal levels of pentobarbital were consumed in the hours prior to death which likely caused her death. It is not possible to determine how Mrs SHUTTLEWORTH came to have this drug in her system, how she was able to access the drug or whether it was taken with the intention of causing her death
