21/10/2025 21/10/2025 WRIGHT, Michael Michael WRIGHT consumed a mixture of illicit drugs prior to collapsing on the path outside his home. He could not be roused on the 6th June 2025 and was taken to Blackburn hospital where he was intubated and ventilated. Despite maximal effort, he died from the effects of multiple drug toxicity and bronchopneumonia causing multi organ failure on the 10th June 2025. Michael had a history of asthma and longstanding drug and alcohol misuse which contributed to his death.

22/10/2025 22/10/2025 CLOUGH, Jaqueline Ann Jacqueline Ann CLOUGH died at her home on the 11th of May 2025 from the effects of a fire she had started deliberately although her intentions could not be better determined on the evidence available

22/10/2025 22/10/2025 RILEY, Earl Earl Fitzritson RILEY died on the 25th November 2024 at the Royal Preston Hospital. Mr RILEY had been admitted to hospital in early November following a heart attack and was due to be discharged when, on the 9th November 2024, he suffered a fall on the ward overnight and fractured his prosthetic hip. This required an operation, following which Mr RILEY did not recover and deteriorated until his death on the 25th November 2024. At the time of his fall, Mr RILEY's buzzer was not reliably working and he was in a bed space far from the toilet. He required assistance to mobilise due to poor eyesight and no moving and handling assessment had been carried out. These factors contributed to Mr RILEY's fall occurring and thus contributed to his death occurring at the time at which it did.
