10/11/2025 13/11/2025 TOWNSEND, Philip Antony Philip Anthony TOWNSEND died on 9 October 2019 at Gordons Working Men's Club, Springfield Street, Morecambe. Mr TOWNSEND has been drinking at the club that afternoon. A fire occurred in the lounge area following a spark or light from the lighter of John James MCCARTNEY in the presence of propane gas. Mr MCCARTNEY tried to take the gas cannister outside after it ignited. The fire doors were not open and accessible, which contributed to Mr TOWNSEND's death. Appropriate measures were not in place to enable evacuation in the event of fire, which contributed to his death. The gas canister was stored inappropriately, which contributed to his death. The risks of fire from the gas cannisters were not managed as they should have been, which contributed to his death.

12/11/2025 12/11/2025 AUSTIN, Peter Leslie Peter Leslie AUSTIN died at his care home on the 4th of October 2024 from an infection. On the 20th of April 2023 he had suffered a fall at his home rendering him quadriplegic which compromised his lung and cardiac functions. He was a known high risk of infection and was hospitalised on a number of occasions prior to his death.
12/11/2025 12/11/2025 DUCKETT, Geoffrey On the 3rd March 2025 Mr Geoffrey DUCKETT was taken to hospital by ambulance with extensive pressure sores and suspected sepsis after falling from his sofa from which he had been unable to move for some time prior. He had a previous leg amputation due to peripheral vascular disease which was causing him pain and affecting his mobility. Care staff had been entering the home four times a day but were not commissioned to care for Mr DUCKETT. They observed Mr DUCKETT's deterioration and self-neglect but the necessary steps to intervene and escalate were not taken. Mr DUCKETT died from sepsis caused by an infected sacral ulcer on the 5th March 2025 at Lancaster Infirmary Lancaster which was contributed to by neglect.
