TOWNSEND, Philip Antony
|
Philip Anthony TOWNSEND died on 9 October 2019 at Gordons Working Men's Club, Springfield Street, Morecambe. Mr TOWNSEND has been drinking at the club that afternoon. A fire occurred in the lounge area following a spark or light from the lighter of John James MCCARTNEY in the presence of propane gas. Mr MCCARTNEY tried to take the gas cannister outside after it ignited. The fire doors were not open and accessible, which contributed to Mr TOWNSEND's death. Appropriate measures were not in place to enable evacuation in the event of fire, which contributed to his death. The gas canister was stored inappropriately, which contributed to his death. The risks of fire from the gas cannisters were not managed as they should have been, which contributed to his death.
