03/11/2025 03/11/2025 WILSON, Julie Julie WILSON died on the 5th July 2025 at the Royal Blackburn Hospital as a consequence of a cerebral vascular event. Mrs WILSON suffered a stroke in May of 2025 following which she was discharged to a rehabilitation ward until it became apparent that her left lower limb had an incomplete blood supply. She underwent surgery for this difficulty following which her death deteriorated and she did not recover. The need for surgery was due to an underlying natural disease process. 
04/11/2025 04/11/2025 DAVIES, Martin James Edwin Martin DAVIES was admitted to Albion Mill Intermediary care on the 5th December 2024 for rehabilitation following a type 2 myocardial infarction secondary to a chest infection. Mr DAVIES had his catheter changed due to a blockage on the 14th December 2024. Shortly after insertion, the catheter balloon migrated from the bladder into the urethra causing pain and bleeding. This placed stress on Mr DAVIES' physiological reserves which precipitated a cardiac arrest and resuscitation was commenced and an ambulance called. Despite resuscitation efforts, Mr DAVIES died on the 15th December 2024 at Albion Mill Intermediate Care Unit in Blackburn. Mr DAVIES had significant underlying coronary artery disease which contributed to his death and was immobile as a result of previous traumatic spinal fractures. 
05/11/2025 05/11/2025 BRIGGS, Ian Christopher On 30th October 2024 Ian Christopher BRIGGS was admitted via ambulance to Royal Preston Hospital from Meadowfield Care Home, Leyland, where he had been receiving rehabilitation with a view to returning home, following femoral by pass surgery. He had a medical background of severe peripheral vascular disease which had become ischaemic lower limb and was seen daily by the District Nurses. On investigation at the A&E dept he was found to have a Grade 4 pressure sore to his sacrum which was hitherto unknown, in addition to the longstanding and deteriorating ulcer to his left foot. Despite surgical debridement and antibiotic treatment, Ian's condition continued to deteriorate due to overwhelming infection materially contributed to by the sacral pressure ulcer, and he was discharged to St. Catherine's Hospice, Preston where he died on 9 December 2024. Personal care provided multiple times a day to Ian afforded opportunities to note and react to the emergence and development of the sacral pressure sore, of which he was known to be at risk, and the failure to do so constituted missed opportunities to treat it.

06/11/2025 06/11/2025 DICKINSON, Harry Harry DICKINSON died on the 20th December 2024 at the Chorley and South Ribble General Hospital as a consequence of injuries he sustained in a fall from a hospital trolley. Mr DICKINSON was suspected to have dementia and had been taken to hospital on the 18th December 2024 due to bleeding in his mouth. The hospital was extremely busy and so Mr DICKINSON was kept in the accident and emergency department overnight where he became increasingly agitated in the environment. Mr DICKINSON required 1:1 care but shortly before handover to day staff, was left unattended. In the time he was unattended, Mr DICKINSON attempted to climb off his trolley and fell, causing the injuries which resulted in his death. His death was contributed to by being left alone when he required 1:1 care at all times.
