18/11/2025 18/11/2025 MOSS, Andrew Paul Andrew Paul MOSS was found deceased on the 20th November 2025 in a home in Stanley, Scotland. It is not possible to determine the direct cause of Andrew's death however it was likely contributed to by drug misuse, acute on chronic bronchitis and emphysema.

20/11/2025 20/11/2025 JACKSON, Joyce Mrs Joyce JACKSON was living with dementia when she collapsed in her care home on the 26 May 2024. She was taken to hospital where an x ray was undertaken which did not show that she was suffering from a fractured neck of femur. She was experiencing delirium which was not fully recognised in the emergency department and she was discharged home with a plan to mobilise her. There was a missed opportunity to return Mrs JACKSON to the hospital the following day and consequently there was a delay in diagnosing the fracture until the 1 June 2024. She underwent surgery on the 3 June 2024 following which, Mrs JACKSON suffered from hypoactive delirium. She did not recover and died on the 16 July 2024 in Blackburn Hospital from hypoactive delirium caused by the fractured neck of femur which had been operated on. It is not possible to determine whether the delay in recognising the fracture contributed to her death. 
20/11/2025 20/11/2025 MANLOVE, Graham Graham MANLOVE who suffered from heart disease and was becoming increasingly frail and less able to care for himself was found dead at his home on the 28th of July 2025. Amphetamine was found in his system.
