Brian HARWOOD died on the 18th January 2022 at Barkingate, Roeburndale, Lancaster as a result of injuries he sustained when he was travelling across a bridge in an all terrain vehicle and the bridge collapsed. The bridge had been constructed by a shooting syndicate around 7 years earlier and was designed without engineering input. As a result, the bridge was inadequately designed and built and did not benefit from appropriate maintenance. The bridge had never been safe for vehicular use. The inadequate design, construction and maintenance of the bridge resulted in its collapse and caused Mr HARWOOD's death.

Carl Anthony MASON was found dead near Ringway Preston on the 24th of June 2025. A cause of death could not be determined due to decomposition.

Mr Peter KIRRAGE was diagnosed with cancer of the hypopharynx in 2024 which was treated with radiotherapy which left him unable to swallow and eat. A percutaneous endoscopic gastrostomy feeding tube was inserted to support his nutrition on the 10 March 2025 and he was discharged the following day. Mr KIRRAGE returned to hospital on the 12th March with a pneumoperitoneum complication of the feeding tube. Whilst being treated for this, he suffered a stroke which was not related to his earlier procedure but which was a terminal event. He was palliated and died at Pendleside Hospice in Burnley on the 22nd March 2025.
